What is MyChart?

MyChart is a secure, online portal designed for you to access portions of your health record online. Through MyChart, you have access to your lab results, immunizations, allergies, medications and follow-up care instructions from clinic, hospital or emergency department visits.

In addition, you may email a question to a Cottage Health outpatient clinic, schedule a clinic appointment, pay your bill online and more.

MyChart is not for medical emergencies. If you have a medical emergency, call 911.

Yes, for access to your Cottage Health or Pacific Diagnostic Laboratories health care information, you will need to sign up for a Cottage Health MyChart account at cottagehealth.org/mychart.

Will I be able to see my Sansum Clinic information in my Cottage Health MyChart account?

No. If you have a Sansum Clinic MyChart account, it will be separate from your Cottage Health MyChart account.

If you have a MyChart account with another health care provider such as Sansum Clinic, you may use MyChart Central to link your accounts and have a single login page for ease of use. MyChart Central is a portal to all of your online MyChart accounts, allowing you access without entering your username and password for each MyChart account. You'll find your health care information in one secure, convenient place. Once you have established your Cottage Health MyChart account, learn more about the MyChart Central portal at mychartcentral.com.

Who can sign up for Cottage Health MyChart?

Any Cottage Health or PDL patients aged 12 or older may sign up for a MyChart account. For patients under the age of 12, parents or legal guardians may obtain proxy access to MyChart records. For patients ages 12 to 18, limited information is available on MyChart including information on allergies, medication allergies, vitals/growth charts and immunization records. At age 18, the patient’s MyChart account will automatically become an adult MyChart account for Cottage Health and Pacific Diagnostic Laboratories.

How do I sign up for Cottage Health MyChart?

You can sign up using the instructions you received from your most recent Cottage Health or Pacific Diagnostic Laboratories visit, or go to cottagehealth.org/mychart if you have a valid email address on file in your health record at Cottage Health, and choose the option that best describes you.

Are minors above age 12 eligible to access their own Cottage Health MyChart account?

Limited information will be available for the teen MyChart access for patients from 12 to 18 years of age, including allergies, medication allergies, vitals/growth charts and immunization records.

Teen access is available once a child reaches age 12. At that age, any previously established proxy access by a parent or legal guardian is restricted. At age 18, their account will automatically become an adult MyChart account with the same access as all other Cottage Health/PDL MyChart accounts.

MyChartCentral is a portal to all of your online MyChart accounts, allowing you to access without entering your username and password for each MyChart account. If you have another MyChart account with another health care provider, you may use MyChartCentral to link your accounts and have a single login page for ease of use. You'll find your health care information in one secure, convenient place. Once you have established your Cottage Health MyChart account, learn more about the MyChartCentral portal at mychartcentral.com.

What is Lucy?

Lucy is a personal health record that gives you a permanent home to organize all your medical information. You can store your past medical records in Lucy. You can also add personal information about your health and choose to share it among all organizations where you receive care. Once you log into your MyChart account, you will see information on how you may populate your own personal health record.

How do I sign up for MyChart Central and Lucy?

Log in to your Cottage Health and Pacific Diagnostic Laboratories MyChart account and locate My Medical Records in the menu. Click on MyChart Central/Lucy and follow the instructions to sign up.

After signing up, you can access MyChart Central and Lucy by going to mychartcentral.com. You can also access MyChartCentral by selecting My Linked Records from the menu in any of your MyChart accounts.

Is MyChart secure?

Security is our top priority. Your MyChart username and password ensure that your information is safe and confidential. If you forget your password, you are able to reset by clicking on the “Forgot Password?” link on the MyChart login screen. After answering security questions, you will be able to select a new password. You should never share your MyChart password.

Call Cottage Health MyChart Services at 805-324-9439 from 9 a.m. to 5 p.m. on Monday through Friday. Please note that MyChart Services is closed on holidays and weekends. Or, you may send an email to mychart@cottagehealth.org.

How do I choose a username and password for my Cottage Health and Pacific Diagnostic Laboratories MyChart account?

Your MyChart password must:

  • be at least eight characters
  • contain at least one uppercase letter
  • contain one lowercase letter
  • contain one number or symbol

Your MyChart username and password ensure that your information is safe and confidential. If you forget your username or password, you are able to reset by clicking on the “Forgot Username” or “Forgot Password?” link on the MyChart login screen. Yes, if you are the parent or legal guardian of a child under the age of 12 or have legal guardianship or power of attorney for another adult that includes access to the patient's health information.

Am I able to message my doctor or health care provider?

You are able to message your doctor or provider if you had an appointment at a Cottage Health outpatient clinic, if it's an appointment that occurred in the last year, or for an upcoming appointment. If you were a patient at a Cottage Health hospital and you need to contact your physician or care team, please call your doctor's office directly.

Why don't I see my physician or provider on the list of people I can message?

If you do not have an upcoming appointment or you haven't been seen recently at a Cottage Health clinic, your physician or provider will not be listed. Please call your clinic directly.

What type of appointments can I schedule at Cottage Health or Pacific Diagnostic Laboratories through Cottage Health MyChart?

For Cottage Health, MyChart scheduling is currently not available. For Pacific Diagnostic Laboratories, you may make an appointment for one of the Patient Service Centers through the website at pdllabs.com.

Can I cancel an appointment through Cottage Health MyChart?

If you have scheduled an appointment at a Cottage Health facility, you will be able to cancel it up to 24-hours in advance through your MyChart account.

How do I know that MyChart information isn't spam or phishing?

Emails from your MyChart account will never ask for your personal information. You will only be notified if you have a new MyChart message in your inbox. You will then need to log in using your user id and password. You should never share this information with anyone.

Why am I not receiving email alerts when there is new information in my Cottage Health MyChart?

When you signed up for MyChart, you may have chosen not to receive any email messages. You may change this setting by logging in to MyChart and go to the “preferences” section.

Why don't I see all of my lab results in Cottage Health/Pacific Diagnostic Laboratories MyChart?

All lab results released into MyChart follow Cottage Health and Pacific Diagnostic Laboratories policies, and state and federal laws. Please note that for the majority of lab results, it will take three days for them to be released to your MyChart record. If you have any questions, ask your health care provider.
